Le lait passe des canalicules aux <strong>canaux galactophores<\/strong>, qui convergent vers le <strong>mamelon<\/strong>, o\u00f9 il est expuls\u00e9 pendant la succion.<\/p>\n<p data-start=\"1178\" data-end=\"1207\">Chaque alv\u00e9ole est constitu\u00e9e de:<\/p>\n<ul data-start=\"1208\" data-end=\"1350\">\n<li data-start=\"1208\" data-end=\"1268\">\n<p data-start=\"1210\" data-end=\"1268\"><strong>Cellules \u00e9pith\u00e9liales (lactocytes)<\/strong>, qui produisent le lait<\/p>\n<\/li>\n<li data-start=\"1269\" data-end=\"1350\">\n<p data-start=\"1271\" data-end=\"1350\"><strong>Cellules myo\u00e9pith\u00e9liales<\/strong>, qui le poussent vers les canaux pendant la contraction<\/p>\n<\/li>\n<\/ul>\n<p data-start=\"1352\" data-end=\"1495\">Tous les lobes et lobules sont entour\u00e9s de <strong>tissu adipeux<\/strong> et s\u00e9par\u00e9s par du <strong>tissu conjonctif fibreux<\/strong> qui sert de structure de soutien.<br \/>\nLa forme des seins, la taille des mamelons ou d&#8217;\u00e9ventuelles l\u00e9g\u00e8res asym\u00e9tries entre les seins <strong>n&#8217;ont aucune incidence sur la capacit\u00e9 d&#8217;allaiter<\/strong>.<\/p>\n<h5 style=\"background-color: #e8eff4; color: white; padding: 5px;\"><strong><span style=\"color: #006271;\">Hormones et changements pendant la grossesse<\/span><\/strong><\/h5>\n<p data-start=\"1699\" data-end=\"1865\">\u00c0 partir de la <strong>m\u00e9narche<\/strong> et, de mani\u00e8re plus intense, pendant la grossesse, le tissu mammaire subit des modifications importantes sous l&#8217;effet de diff\u00e9rentes hormones :<\/p>\n<ul data-start=\"1867\" data-end=\"2101\">\n<li data-start=\"1867\" data-end=\"1929\">\n<p data-start=\"1869\" data-end=\"1929\"><strong>Estrog\u00e8nes<\/strong>: stimulent le d\u00e9veloppement des canaux galactophores<\/p>\n<\/li>\n<li data-start=\"1930\" data-end=\"1999\">\n<p data-start=\"1932\" data-end=\"1999\"><strong>Progest\u00e9rone<\/strong>: favorise l&#8217;augmentation du volume des alv\u00e9oles et des lobes<\/p>\n<\/li>\n<li data-start=\"2000\" data-end=\"2101\">\n<p data-start=\"2002\" data-end=\"2101\"><strong>Prolactine<\/strong>: contribue \u00e0 l&#8217;hypertrophie mammaire et \u00e0 la pr\u00e9paration \u00e0 la lactation<\/p>\n<\/li>\n<\/ul>\n<p data-start=\"2103\" data-end=\"2362\">Pendant la grossesse, on observe une augmentation de la <strong>pigmentation de l&#8217;ar\u00e9ole<\/strong>, de la <strong>taille du mamelon<\/strong> et de la <strong>visibilit\u00e9 des vaisseaux sanguins sous-cutan\u00e9s<\/strong>, autant de signes de l&#8217;adaptation physiologique du sein \u00e0 la fonction d&#8217;allaitement.<\/p>\n<h5 style=\"background-color: #e8eff4; color: white; padding: 5px;\"><strong><span style=\"color: #006271;\">Les phases de la lactation<\/span><\/strong><\/h5>\n<p data-start=\"2403\" data-end=\"2501\">La production de lait maternel, appel\u00e9e <strong>lactation<\/strong>, se divise en quatre phases principales:<\/p>\n<ol data-start=\"2503\" data-end=\"3533\">\n<li data-start=\"2503\" data-end=\"2676\">\n<p data-start=\"2506\" data-end=\"2676\"><strong>Lactogen\u00e8se I<\/strong> <em>(de la moiti\u00e9 de la grossesse au 2e jour post-partum)<\/em><br data-start=\"2580\" data-end=\"2583\" \/>La synth\u00e8se du lait commence, sous contr\u00f4le hormonal. \u00c0 ce stade, le colostrum est produit.<\/p>\n<\/li>\n<li data-start=\"2678\" data-end=\"2915\">\n<p data-start=\"2681\" data-end=\"2915\"><strong data-start=\"2681\" data-end=\"2699\">Lactogen\u00e8se II<\/strong> <em>(du 3e au 8e jour apr\u00e8s l&#8217;accouchement)<\/em><br data-start=\"2736\" data-end=\"2739\" \/>On observe une augmentation significative de la production de lait, souvent accompagn\u00e9e d&#8217;engorgement mammaire. Le contr\u00f4le est encore endocrinien et s&#8217;ach\u00e8ve dans les 30 \u00e0 40 heures suivant l&#8217;accouchement.<\/p>\n<\/li>\n<li data-start=\"2917\" data-end=\"3284\">\n<p data-start=\"2920\" data-end=\"3284\"><strong>Galactopo\u00ef\u00e8se<\/strong> <em>(du 9e jour jusqu&#8217;au d\u00e9but de l&#8217;involution)<\/em><br \/>\nLa production de lait est principalement r\u00e9gul\u00e9e par des <strong>stimuli locaux<\/strong>, en fonction de la fr\u00e9quence et de l&#8217;efficacit\u00e9 de la succion. Ce m\u00e9canisme de contr\u00f4le <strong>autocrine<\/strong> suit le principe de l&#8217;offre et de la demande. La production se stabilise vers la <strong>4e \u00e0 la 6e semaine<\/strong> apr\u00e8s l&#8217;accouchement.<\/p>\n<\/li>\n<li data-start=\"3286\" data-end=\"3533\">\n<p data-start=\"3289\" data-end=\"3533\"><strong>Involution<\/strong> <em>(environ 40 jours apr\u00e8s la derni\u00e8re t\u00e9t\u00e9e)<\/em><br data-start=\"3346\" data-end=\"3349\" \/>Elle commence lorsque l&#8217;enfant r\u00e9duit ses t\u00e9t\u00e9es ou commence \u00e0 manger des aliments compl\u00e9mentaires. L&#8217;accumulation du <strong>facteur inhibiteur de la lactation (FIL)<\/strong> r\u00e9duit progressivement la production.<\/p>\n<\/li>\n<\/ol>\n<h5 style=\"background-color: #e8eff4; color: white; padding: 5px;\"><strong><span style=\"color: #006271;\">R\u00f4le des hormones dans le maintien de l&#8217;allaitement<\/span><\/strong><\/h5>\n<p data-start=\"3603\" data-end=\"3769\">Apr\u00e8s l&#8217;accouchement, les taux <strong>d&#8217;\u0153strog\u00e8nes et de progest\u00e9rone<\/strong> diminuent rapidement, permettant \u00e0 la <strong>prolactine<\/strong> et \u00e0 <strong>l&#8217;ocytocine<\/strong> de jouer un r\u00f4le central:<\/p>\n<ul data-start=\"3771\" data-end=\"4305\">\n<li data-start=\"3771\" data-end=\"3990\">\n<p data-start=\"3773\" data-end=\"3990\"><strong>Prolactine<\/strong>: elle stimule les cellules \u00e9pith\u00e9liales des alv\u00e9oles \u00e0 produire du lait. Elle suit un rythme circadien, avec des valeurs plus \u00e9lev\u00e9es pendant la nuit. L&#8217;allaitement nocturne favorise donc une plus grande production de lait.<\/p>\n<\/li>\n<li data-start=\"3992\" data-end=\"4305\">\n<p data-start=\"3994\" data-end=\"4305\"><strong>Ocytocine<\/strong>: elle induit la contraction des cellules myo\u00e9pith\u00e9liales autour des alv\u00e9oles, favorisant l&#8217;\u00e9coulement du lait vers le mamelon (r\u00e9flexe d&#8217;\u00e9jection). Sa s\u00e9cr\u00e9tion est stimul\u00e9e par le contact physique, les \u00e9motions positives, les c\u00e2lins et les caresses, mais elle peut \u00eatre <strong>inhib\u00e9e par le stress, la douleur, la nicotine ou l&#8217;alcool<\/strong>.<\/p>\n<\/li>\n<\/ul>\n<h5 style=\"background-color: #e8eff4; color: white; padding: 5px;\"><strong><span style=\"color: #006271;\">Facteur inhibiteur de la lactation (FIL)<br \/>\n<\/span><\/strong><\/h5>\n<p>Le <strong>FIL<\/strong> est une substance produite localement par les alv\u00e9oles qui r\u00e9gule la production de lait. Lorsque le sein n&#8217;est pas vid\u00e9 r\u00e9guli\u00e8rement, le FIL s&#8217;accumule et inhibe la synth\u00e8se du lait. Seule une \u00e9limination fr\u00e9quente et efficace du lait, par des t\u00e9t\u00e9es r\u00e9guli\u00e8res, un tirage manuel ou l&#8217;utilisation d&#8217;un tire-lait, peut contrer cette inhibition et maintenir une production ad\u00e9quate.<\/p>\n<p><em>L&#8217;allaitement maternel est un processus complexe, r\u00e9gul\u00e9 par un syst\u00e8me anatomique et hormonal sophistiqu\u00e9.
