No hormone or system acts in isolation: the <strong>growth process<\/strong> depends on the integration between <strong>the central nervous system, endocrine system, metabolism and nutritional status<\/strong>.<\/p>\n<p>Among the <strong>main mechanisms<\/strong> involved are:<\/p>\n<ul>\n<li>the hypothalamus-pituitary axis;<\/li>\n<li>growth hormone (GH);<\/li>\n<li>the insulin-like growth factor 1 (IGF-1);<\/li>\n<li>thyroid hormones;<\/li>\n<li>energy metabolism;<\/li>\n<li>the availability of essential nutrients.<\/li>\n<\/ul>\n<p>The activity of these systems varies physiologically during childhood, contributing to the different growth rates observed in various stages of development.<\/p>\n<p>&nbsp;<\/p>\n<h5 style=\"background-color: #e8eff4; color: white; padding: 5px;\"><strong><span style=\"color: #006271;\">The GH-IGF-1 axis<\/span><\/strong><\/h5>\n<p>The main endocrine system involved in growth after birth is represented by the <strong>hypothalamus-pituitary-GH-IGF-1 axis<\/strong>.<br \/>\nThe hypothalamus regulates the secretion of growth hormone (GH) through the balance between two hormones:<\/p>\n<ul>\n<li>the <strong>GHRH (Growth Hormone Releasing Hormone)<\/strong>, which stimulates its release;<\/li>\n<li><strong>somatostatin<\/strong>, which instead inhibits its secretion.<\/li>\n<\/ul>\n<p>Once released from the pituitary gland, GH acts directly on various tissues and mainly stimulates the production of <strong>insulin-like growth factor 1 (IGF-1)<\/strong> in the liver and peripheral tissues.<br \/>\nIGF-1 represents one of the main <strong>mediators of postnatal growth<\/strong> and contributes to:<\/p>\n<ul>\n<li>promoting the proliferation of chondrocytes in growth cartilage;<\/li>\n<li>supporting the growth of long bones;<\/li>\n<li>increasing protein synthesis;<\/li>\n<li>promoting the development of muscle mass;<\/li>\n<li>supporting tissue maturation.<\/li>\n<\/ul>\n<p>The activity of the GH-IGF-1 axis is not constant throughout childhood, but varies depending on age, nutritional status, body composition and biological maturation.<\/p>\n<p>&nbsp;<\/p>\n<h5 style=\"background-color: #e8eff4; color: white; padding: 5px;\"><strong><span style=\"color: #006271;\">The role of thyroid hormones<\/span><\/strong><\/h5>\n<p>Also <strong>thyroid hormones<\/strong> play a fundamental role in the regulation of growth and development.<br \/>\nTheir action affects numerous <strong>physiological processes<\/strong>, including:<\/p>\n<ul>\n<li>the control of cellular metabolism;<\/li>\n<li>the maturation of the central nervous system;<\/li>\n<li>skeletal development;<\/li>\n<li>the regulation of tissue response to growth stimuli.<\/li>\n<\/ul>\n<p>During the first years of life, adequate production of thyroid hormones is particularly important for correct neurological development, as well as for normal height growth.<\/p>\n<p>&nbsp;<\/p>\n<h5 style=\"background-color: #e8eff4; color: white; padding: 5px;\"><strong><span style=\"color: #006271;\">Nutrition and metabolism<\/span><\/strong><\/h5>\n<p>Growth requires a continuous supply of energy and nutrients. To support the formation of new tissues, the organism must maintain a <strong>balance between energy availability, protein synthesis and cellular metabolism<\/strong>.<br \/>\nDuring phases of more rapid growth <strong>the requirements for<\/strong> increase:<\/p>\n<ul>\n<li>energy;<\/li>\n<li>proteins;<\/li>\n<li>essential fatty acids;<\/li>\n<li>vitamine;<\/li>\n<li>minerals and trace elements.<\/li>\n<\/ul>\n<p>An adequate nutritional intake allows for the support of <strong>cell proliferation<\/strong>, <strong>organ maturation<\/strong> and <strong>nervous system development<\/strong>. Conversely, malnutrition conditions or chronic pathologies can negatively influence growth rate.<\/p>\n<p>For this reason, pediatric assessment always considers growth in the context of the child&#8217;s overall nutritional status.<\/p>\n<p>&nbsp;<\/p>\n<div style=\"background: #E8EFF4; padding: 20px;\">\n<h5 style=\"background-color: #e8eff4; color: white; padding: 5px;\"><em><strong><span style=\"color: #006271;\">Why growth regulation changes during childhood<\/span><\/strong><\/em><\/h5>\n<p><span style=\"color: #006271;\"><em>The mechanisms that regulate growth do not maintain the same importance throughout development. At the same time, profound changes occur in <strong>body composition<\/strong>, in <strong>organ maturation<\/strong> and in the <strong>development of neurological functions<\/strong>.<\/p>\n<p>Each phase of childhood is characterized by a <strong>different balance between somatic growth and functional maturation<\/strong>. In the first months of life, rapid body growth prevails, while later, motor, cognitive and relational development take on an increasingly important role. For this reason, the interpretation of growth cannot be based on a single anthropometric parameter, but must consider the set of biological processes that accompany the child&#8217;s development.<\/p>\n<p>&nbsp;<\/p>\n<h5 style=\"background-color: #e8eff4; color: white; padding: 5px;\"><strong><span style=\"color: #006271;\">Conclusions<\/span><\/strong><\/h5>\n<p>Child growth is the result of dynamic interaction between endocrine, metabolic, nutritional and neurological factors. The coordinated activity of the GH-IGF-1 axis, thyroid hormones and the mechanisms that regulate metabolism allows for normal growth and progressive maturation of organs and tissues.<\/p>\n<p>Growth regulation varies physiologically in different stages of childhood, reflecting changes in the biological systems that support the development of the organism.
