Distribution depends on <strong>lipophilicity<\/strong> and <strong>plasma protein binding<\/strong>; <strong>lipophilic and low protein-bound<\/strong> drugs tend to pass into the milk more easily. Hepatic metabolism can generate <strong>active or inactive metabolites<\/strong>, some of which may affect the newborn if present in milk. Understanding drug biodynamics, maternal immune response and the transfer of IgG and IgA antibodies allows for the safe management of fever and influenza in pregnant women and new mothers, while simultaneously protecting the fetus and the newborn.<\/p>\n<p>The main scientific sources and guidelines supporting these recommendations include the <strong>Centers for Disease Control and Prevention<\/strong> (<strong>CDC, 2025<\/strong>), which provides guidance on the management of influenza in breastfeeding women and the use of safe antivirals, the <strong>American Academy of Pediatrics <\/strong>(<strong>AAP, 2025<\/strong>) which recommends the continuation of breastfeeding and the use of drugs compatible with breast milk, the <strong>World Health Organization <\/strong>(<strong>WHO, 2024<\/strong>) for clinical guidelines on seasonal influenza, and the <strong>LactMed database of the National Institutes of Health<\/strong> (<strong>NIH<\/strong>), which documents drug transfer into milk and safe exposure levels for the infant.<\/p>\n<p>&nbsp;<\/p>\n<h5 style=\"background-color: #e8eff4; color: white; padding: 5px;\"><strong><span style=\"color: #006271;\">Drug biodynamics and transfer into breast milk<\/span><\/strong><\/h5>\n<p>Drug biodynamics is the <strong>study of the processes by which an active ingredient exerts its action in the body<\/strong>, including cellular, tissue and systemic effects.
